Note: That a VPS host accepts Bitcoin does not automatically mean that they allow anonymous registrations! Please check with an individual host about their policy on this, before providing fake details.
Another thing to note is that not all providers allow you to select Bitcoin from the ordering system. If the option is not offered during ordering, ask the support department.

Update, April 10, 2015: It looks like the page has been deleted from the Bitcoin wiki. I'll be keeping an eye on the wiki, and link the new page if it ever comes to exist again.
Removed providers:
- Vultr - Now require adding a creditcard or PayPal account before being able to use Bitcoin, which defeats the point.
- UrDN - Servers and equipment seized by the Security Service of Ukraine (SBU). Unclear whether service will return. Will be re-added to the list if/when they do.
- Shard Hosting - No longer in business.
- HeroicVPS - No longer in business.
- Centarra - Removal from list requested. Update, August 6, 2015: Closing business on September 1, 2015.
- BlueVM - Seemingly dead in the water; many reports of unanswered support tickets, extended downtime, etc. Shows symptoms of going out of business. Update, August 6, 2015: Acquired by ChicagoVPS, but reports of problems with the service and support remain.
- ViralVPS - Seemingly no longer in business, domain 404s.
